User Level and Team Level App Access Confusion

Users, specifically administrators, have noticed confusion in the application when revoking app access from a team / user, but users still have direct user / team. The successful revocation is reported, but the user still has access to the app.

What is happening or what is needed
A way to make it less confusing about the end state of the action. After seeing a successful message, I presume the users no longer have access to the app that they still do by some other means.
Walk us through it step by step
1. Grant a user an app directly (Admin → Users → user → Apps → Grant). 2. Create a team, add the same user, and grant the same app to the team. 3. On the user's Apps page, select that app and click Revoke. 4. Observe: green 'App access updated successfully' toast, but the Access badge stays on.
